Hospital discharge and NHS support

NHS Hampshire and Isle of Wight ICB coordinates health and social care across the county, working alongside Hampshire County Council and the Southampton and Portsmouth City councils on hospital discharge planning. Families arranging care after a hospital stay can set up live-in care to support a safe return home, regardless of whether your family member has been in the Southampton General or Queen Alexandra in Portsmouth.

As a CQC regulated carer provider, we can arrange full costs and available live-in carers for:

Condition-specific care

Dementia or any other condition-specific care

Reablement care

Postoperative or any other injury-related care

General care

Overnight, companionship or disability care

CQC regulation

Hometouch's managed live-in care service is regulated by the Care Quality Commission and rated Good. Inspectors assess how safe, effective, caring, responsive and well led a service is, and the full report is available on the CQC website.

Paying for live-in care in Hampshire

Hampshire County Council carries out a care needs assesment for most of the county, while Southampton City Council and Portsmouth City Council assess residents in those two areas as separate unitary authorities. As with the rest of England, anyone with assets above £23,250 pays for their own care in full, with means-tested support available below that threshold and full support below £14,250.
